PRE-OWNED FORMULAS ONLY:

I understand that by submitting this transfer request, I am requesting transfer of the boat’s owner registration information only. To transfer policy holder information for any remaining warranty on the 5-year Formula Guard extended protection plan, I must contact the appropriate Formula Guard carrier within 30 days of the date of retail purchase. If my boat qualifies, the Formula Guard administrator will send a transfer form.
